Position Summary: The Resident Service Coordinator, as part of the broader Harborlight Homes team and reporting to the Senior Resident Service Coordinator, will provide on-site support and guidance to residents, assists in retaining tenancy, promoting resident independence, and acting as an advocate for residents and affordable housing in general. The Resident Service Coordinator will contribute to the success and functioning of the Resident Services team in support of the mission, vision and goals of Harborlight Homes.
Responsibilities/Essential Functions:
- Conduct thorough intake processes to orient residents to housing, provide general care management during their tenancy, and assist in providing referral services for residents needing or requesting assistance while monitoring ongoing provision of services from community agencies.
- Coordinate and manage programs and projects for residents designed to promote health, wellness, and education. Inform and update residents and families on available resources.
- Advocate for and educate residents on the following including but not limited to service availability, application processes, client rights, as appropriate and in alignment with company policy.
- Develop and update care plans in coordination with community assessment services and bi-annual assessments.
- Conduct bi-annual HUD resident assessments in adherence with an established timeline.
- Proactively establish and cultivate relationships with residents, team members, the broader elder communities, and other social service and community service agencies to improve upon the experience and services available through Harborlights Homes.
- Meet and communicate with resident service providers when needed or requested with appropriate approvals.
- Act on behalf of residents at their request and/or support communication with community agencies and service providers maintaining privacy and confidentiality.
- Provide support and crisis intervention on behalf of residents and their families.
- Collaborate with onsite property management, maintenance, and social services teams as necessary to identify residents in need, conduct compliant apartment inspections, and resolve challenges or violations.
- Assists in preparing and submitting reports, gather and assess data, and maintain up to date information in relevant databases and filing systems including referrals, resources, and outcomes ensuring privacy and confidentiality of resident files and information.
- Maintain relevant or required training, certifications, or education as needed to meet regulatory standards.
Qualifications:
- Bachelor’s Degree in Social Work, Psychology or Counseling or related field preferred or 2-3 years related work experience in social service environment
- Prior experience working with diverse populations
- Competency with Microsoft Office and general computer proficiency
Requirements:
- Access to reliable transportation to travel locally in the Beverly area or North Shore
Physical Demands:
- Required to sit for long periods of time
- Infrequent light physical effort required
